Position Overview
The IDD Crisis Intake Coordinator serves as the point of entry for publicly funded intellectual and developmental disability (IDD) programs across all eight counties in the BTCS catchment. The coordinator enrolls eligible individuals on an expedited timeframe into Medicaid/GR programs including ICF/IID, Community First Choice (CFC), GR Community Services, and Autism Services. The role provides crisis intervention/prevention supports, advocacy services, and links to job readiness activities through the Employment First project.
Responsibilities- Complete eligibility screenings for ICF, CFC, Autism, and General Revenue streams from multiple referring agencies.
- Admit clients to GR, ICF/MR, CFC services, Autism services, and crisis intake support.
- Assess needs, develop service plans, facilitate service linkages, and monitor effectiveness.
- Advocate and link individuals interested in employment activities through the Employment First project.
- Participate in interagency planning and CRCG meetings to bridge gaps and avoid duplication of services.
- Utilize initiative, independent judgment, assertive professional intervention, and frequent telehealth services.
- Work with families and service providers with sensitivity to cultural differences and minimal supervision.
- Maintain flexible hours and schedule to meet family needs and comply with HHSC Principles and Medicaid requirements.
- Bachelor’s degree or higher in social, behavioral, or human service field (e.g., psychology, social work, nursing, counseling, sociology, human development, gerontology, educational psychology, education, criminal justice).
- Valid Texas driver’s license and maintain approved driver’s status throughout employment.
- At least one year case management or service coordination experience working with individuals diagnosed with Intellectual or Developmental Disability.
- Knowledge of appropriate community resources and experience with IDD programs and Medicaid Waiver programs.
- Familiarity with the Smart Care program.
- Master’s Degree.
- Bilingual.
Remote serving eight counties (Bastrop, Burnet, Caldwell, Fayette, Gonzales, Guadalupe, Lee, Williamson). Some travel may be required within the counties.
Hourly Salary Range$21.46 – $30.21
Salary Increment Options- Up to $2.00/hour – Prior experience at a community center in a similar position.
- $1.00/hour – Master’s Degree in HS.
- Up to $4.00/hour – Tenure with BTCS.
