Overview
The Bluebonnet Trails Community Services (BTCS) Assertive Community Treatment (ACT) Team Intensive Case Manager is responsible for providing outstanding person-centered intensive case management services as part of the ACT Team to a small caseload of adults with current or historical substance use treatment needs and a diagnosed severe and persistent mental health disorder, who have experienced multiple hospitalizations. ACT is a model for comprehensive, integrated, community-based services based on a multidisciplinary team to provide psychiatric treatment, social services and rehabilitation.
This is an evidence-based treatment originally developed in the 1970s as a hospital without walls to achieve better functioning in the community for people with severe mental illness.
This position reports directly to the Behavioral Health Services Manager and requires the development and maintenance of effective, professional working relationships with stakeholders and other community service providers. The position has flexible hours dependent upon the needs of the caseload and offers the option to work remotely to complete required administrative tasks and/or when televideo services are deemed appropriate by the supervisor.
Responsibilities- Regularly meets with individuals in their homes, community, hospitals, crisis units, criminal justice settings, and at BTCS clinics to provide case management and rehabilitative skills training services.
- Meet people where they are in the community to achieve outcomes that stabilize mental health symptoms, reduce risk of recidivism, increase awareness of and participation with community and natural supports, develop self-advocacy, and assist with participation in independent behavioral health services.
- Be an integral part of a strong team with frequent updates on the recovery process of individuals in service.
- Receive unique and specialized training and professional development focused on services to a small caseload of individuals with multiple hospitalizations and on a state-of-the-art electronic medical record.
- Demonstrate innovative thinking, patience, resiliency, advocacy, service coordination, mental health assessment and evaluation, collaborative person-centered recovery planning, and dual-diagnosis treatment.
- Work effectively both independently and as part of a team; prioritize needs during crisis situations; engage in regular meetings and training opportunities; balance self-care; maintain a positive attitude and perform duties as assigned.

Compensation Details- Up to $4.00/hour – Tenure with BTCS
- $0.75/hour – Bilingual
- $Up to 2.00/hour – prior experience in a similar role with a community center in Texas
- $1.00/hour – Masters Degree in Human Services
- $1.00/hour – Advanced Licensure
- $0.50/hour – LCDC Licensure
Eligible for Quarterly Service Delivery Award
Minimum QualificationsA bachelor’s degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, or early childhood education/intervention; education and experience in behavioral healthcare, intensive community treatment, and substance use treatment; and a valid Texas Driver’s License meeting BTCS driving requirements throughout employment.
- If the degree is not in a Human Services field, a minimum of 24 college credits in a Human Services field is required.
- Previous experience working with an Assertive Community Treatment program, behavioral health, and/or case management.
- Advanced degree in a Human Services field.
- Licensed Practitioner (LCDC, LCSW, LMFT, LPC).
