Job Description
The Bluebonnet Trails Community Services (BTCS) Assertive Community Treatment (ACT) Team Intensive Case Manager is responsible for providing outstanding person-centered intensive case management services as part of the ACT Team to a small caseload of adults who have current or historical substance use treatment needs, a diagnosed severe and persistent mental health disorder, and who have experienced multiple hospitalizations. ACT is a model for comprehensive, integrated, community-based services based on a multidisciplinary team to provide psychiatric treatment, social services, and rehabilitation.
This evidence-based treatment, initially developed in the 1970s as a ‘hospital without walls,’ aims to improve community functioning for people with severe mental illness, enable community living, empower individuals, and reduce inpatient psychiatric recidivism.
The position reports directly to the Behavioral Health Services Manager and collaborates with stakeholders and other community service providers. Hours are flexible, dependent upon caseload needs, and the role offers the option to work remotely for administrative tasks or televideo services when appropriate.
Responsibilities include regularly meeting with individuals in homes, community settings, hospitals, crisis units, criminal justice environments, and BTCS clinics to deliver case management and rehabilitation skills training. Services focus on stabilizing mental health symptoms, reducing risk of recidivism, increasing engagement with community supports, fostering self-advocacy, and facilitating participation in independent behavioral health services. The clinician participates in frequent team connects, receives specialized training, and manages a state-of-the-art electronic medical record.
The role requires innovative thinking, patience, resilience, advocacy, service coordination, mental health assessment, collaborative recovery planning, and dual-diagnosis treatment. The candidate must work independently and as part of a team, prioritize crisis situations, engage in regular meetings and training, maintain self-care, and perform duties with a positive attitude.

Hourly Salary Range: $22.21 – $36.13
Differential Detail- Up to $4.00/hour – Tenure with BTCS
- $0.75/hour – Bilingual
- Up to $2.00/hour – prior experience in a similar role with a community center in Texas
- $1.00/hour – Masters Degree in Human Services
- $1.00/hour – Advanced Licensure
- $0.50/hour – LCDC Licensure

QualificationsMinimum Qualifications:
- A bachelor’s degree from an accredited college or university with a minimum of 24 college credits equivalent to a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistant, gerontology, special education, educational psychology, early childhood education/intervention.
- Education and experience in behavioral healthcare, intensive community treatment, substance use, abuse, and dependency treatment.
- Must have a valid Texas Driver's License and meet BTCS driving requirements, maintaining approved driving status throughout employment.
- Previous experience working with an Assertive Community Treatment program, behavioral health, and/or case management.
- Advanced degree in Human Services field.
- Licensed Practitioner (LCDC, LCSW, LMFT, LPC).
Salary Amount: $42,515.00 – $52,395.00 per year.
