Qualifications
  • You are a board-certified APRN who is passionate about serving underserved populations and removing barriers to care.
  • Comfortable providing care in person and virtually; capable of growing a practice with patient participation in decision-making for treatment in an outpatient setting.
  • Mission-focused and committed to improving access to quality healthcare.
  • Proactive and thrives in a fast-paced clinical setting.
  • Able to work independently in a group setting while seeking support and consultation from colleagues.
  • Ability to collaborate with community partners and emergency medical/psychiatric service providers to ensure patient safety if higher levels of care are needed.
  • Strong computer/technology skills to document in an electronic health record, conduct telemedicine visits, and communicate with colleagues virtually.
  • Bilingual English/Spanish language skills preferred.

All LSCC employees function as members of a collaborative health care team to create and maintain a Patient Centered Medical Home, emphasizing excellent communication, support for patient self-management, and culturally competent care supported by information technology.
