An exciting opportunity has arisen for a motivated and compassionate Band 6 Cardiac Rehabilitation Nurse to join our dynamic cardiacrehabilitation team on a full-time basis.

This role is community based, with opportunities for hospital in-reach, supporting patients following myocardialinfarction, PCI, and CABG, from admission through to long-term management andsecondary prevention. Experience in, or a strong interest in, heart failuremanagement is desirable.

Main duties of the job
  • You will play a key role in delivering evidence-based cardiacrehabilitation to patients .
  • Working autonomously and as part of amultidisciplinary team, you will support patients to optimise their physical,psychological, and lifestyle outcomes following a cardiac event or diagnosis.
  • Assess, plan, implement, and evaluate individualised cardiacrehabilitation programmes
  • Provide specialist nursing input for patients post cardiacevent, including education, symptom monitoring, and self-management support
  • Deliver hospital in-reach services, engaging patients earlyin their recovery journey
  • Lead and contribute to exercise, education, and lifestylemanagement sessions in the community
  • Work collaboratively with cardiology teams, GPs, alliedhealth professionals, and voluntary services
  • Support audit, service development, and quality improvement initiatives

Person Specification Qualifications
  • Registered Nurse (Adult) with current NMC registration.
  • First degree in Nursing (or equivalent qualification and experience).
  • Teaching and/or Assessing qualification (e.g. Mentorship, Practice Assessor, or equivalent).
  • Non-Medical Prescriber qualification (or willingness to undertake training as required by the service).
  • Post-basic qualification or completion of a cardiac-related course (e.g. coronary care, cardiac rehabilitation, or heart failure).
  • Completion of BACPR-accredited Cardiac Rehabilitation Course and/or Heart Manual Facilitation Training.
  • Evidence of or willingness to undertake specialist clinical training relevant to the role, such as:

    ECG recording and interpretation:
  • Venepuncture and cannulation and Exercise testing and risk stratification
Experience
  • Registered Nurse (NMC) with a minimum of two years post-registration experience, including significant experience in cardiac care.
  • Sound understanding of a wide range of cardiac conditions and their management, including acute coronary syndromes, heart failure, and post-interventional care.
  • Proven experience in the delivery of cardiac rehabilitation programmes.
  • Experience in providing community-based cardiac care, demonstrating ability to deliver services across acute and community settings.
  • Experience in the management of heart failure patients, including optimisation of therapy and ongoing monitoring.
  • Evidence of involvement in the development and implementation of clinical protocols, guidelines, and clinical audits to support service improvement and quality assurance.
  • Demonstrates commitment to continuing professional development (CPD) and evidence of maintaining and enhancing clinical competence.
  • Understanding of the principles and challenges of community-based working, including multidisciplinary and cross-sector collaboration.
  • Working knowledge of local policies, procedures, and practices relevant to cardiac care, and awareness of the national NHS strategy, NICE guidance, and BACPR standards and core components for cardiac rehabilitation.
