General Purpose
Under the direct supervision of the shift L.P.N. and indirect supervision of the Assistant Director of Health Services, the Certified Nursing Assistant (CNA) provides care and assistance to the residents of Wellington Court at St George Village with activities of daily living including dressing, grooming, hygiene, bathing, mobility, meals, activities and ongoing monitoring of resident's status and abilities. Communicates with co-care partners and supervisors.

Customer Service: Responsible for effectively meeting customer needs by caring out established customer service / guest relations protocols of SGV. Meeting the goal of having a very high degree of responsive and friendly customer service for all customers and taking responsibility for customer satisfaction.
Safety: Follows departmental safety practices. Works safely and contributes to a safe work environment for co-workers. Reports unsafe conditions and corrects in a timely manner.

Essential Duties:
- Responsible for assisting residents with activities of daily living including: dressing, bathing, grooming, hygiene and toileting.
- Responsible for providing assistance with mobility, including use of a mechanical lifting equipment.
- Responsible for providing supervision at meal time, serving meals in the dining room, and eating assistance.
- Responsible for having knowledge of special or modified diets for specific residents
- Responsible for communicating with other healthcare providers to promote continuity of care
- Hospice, private caregivers and therapy
- Responsible for implementing individual resident care plans:
- Assists with AOL's
- Provides total care when necessary
- Transports residents within the community for planned activities
- Provides resident care according to resident preferences
- Performs resident - appropriate activities as indicated
- Responsible for monitoring resident's condition and reporting any changes to the charge nurse
- Responsible for implementing CN.A care plan for newly admitted resident.
- Responsible for monitoring and maintaining appropriate resident records for accuracy and regulatory compliance.
- Responsible for Implementing and following established polices and procedures of Wellington Court ..
- Performs shift rounds with oncoming and/ or off going shift
- Communicates with nurse at end of shift related to any resident care issues
- Performs shift duties as assigned
