Program and Role Summary
Wayfinder’s Kinship Support Services Program is a kinship navigation service for grandparents, extended family members or close family friends who step up to raise children so they avoid the trauma of separation from their birth family. The program also assists kinship families that form voluntarily, without the assistance of a child welfare agency. Our program offers counseling, assistance with basic needs, support groups and workshops, respite resources and more.
Kinship families are more stable, and children maintain their family connections, which produces better long-term outcomes.
The primary focus of the Social Worker
, is to work with post adoptive and post permanency families to stabilize and strengthen client families. To accomplish this, the Social Worker II will provide engagement, assessment, goal planning, skill building, referrals, advocacy, and support during crisis. Assist families in locating funding for services where possible. Accurately document all services provided, assessments, meetings, decisions, and reports. The social worker II will work cooperatively and collaboratively with team members and community partners.
Primary Responsibilities:
- Promotes the Continuous Quality Improvement (CQI) process by identifying quality indicators and opportunities for departmental and organizational process improvement.
- Reports any incidents/accidents according to incident reporting procedures and protocols, within designated timelines as outlined in the reporting procedures. Ensures staff is knowledgeable of and adhere to the reporting procedures and protocols.
- Maintains confidentiality of all pertinent client information.
- Promotes a healthy organizational culture based on trust, respect, empowerment and accountability.
- Ensures sensitivity to the service population’s cultural and socioeconomic characteristics.
- Maintain professional integrity and ethics.
- Work cooperatively and collaboratively with others
- Refer families for clinical support to clinicians on the Permanency Project team.
- Other duties as outlined in the position description.
Qualifications
Education and Experience:
- A master’s degree in social work, Psychology, Sociology and/or Communication is required.
- Prior experience in a case management position or equivalent experience
- Knowledge of crisis intervention and the unique needs of kinship care and adoptive families
- Strong knowledge of and ability to deliver trauma-informed practice
- Ability to work in MS Word, Outlook and Excel at intermediate level
- Proficient in using virtual video technologies and have stable internet service with privacy to meet with clients virtually
- Work in adoptions preferred.
