Pine Creek Care Center is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our team on the AM shift (6:30AM - 2:30PM). This role is ideal for an LVN who enjoys hands-on resident care, teamwork, and leading CNAs in delivering exceptional clinical services in a long-term care setting.

As an LVN at Pine Creek Care Center, you will provide direct nursing care to residents while supervising nursing assistants to ensure safe, respectful, and high-quality care. You will work closely with physicians, families, and interdisciplinary team members to support residents' health, comfort, and well-being.

• Provide direct nursing care in accordance with resident care plans and physician orders
• Supervise and support CNAs during daily care activities
• Administer medications and treatments safely and accurately
• Complete timely and accurate charting and clinical documentation
• Monitor resident conditions and promptly report changes
• Admit, transfer, and discharge residents as needed
• Communicate effectively with residents, families, physicians, and leadership
• Maintain compliance with state, federal, and facility regulations
• Participate in quality assurance, infection control, and safety initiatives
• Foster a calm, respectful, and positive environment for residents and staff

• Current, active, and unencumbered LVN license in the state
• Current CPR certification
• Long-term care experience preferred
• Knowledge of Point Click Care  (PCC) preferred
• Strong clinical, organizational, and communication skills
• Commitment to resident dignity, safety, and confidentiality
