Overview
Job Title: Occupational Therapist (OT) – Contract to Permanent
Location: Skilled Nursing Facility – California
Employment Type: Contract with Opportunity for Permanent Placement
Position SummaryWe are seeking a compassionate and skilled Occupational Therapist (OT) to join our interdisciplinary rehabilitation team at a Skilled Nursing Facility in California. This position will begin as a contract role
, with the potential to transition into a permanent, full-time position based on performance and facility needs.
The Occupational Therapist will evaluate residents, develop individualized treatment plans, and provide therapy services to help patients regain independence and improve quality of life.
Key Responsibilities- Evaluate residents’ functional abilities and limitations related to activities of daily living (ADLs)
- Develop, implement, and modify individualized treatment plans in accordance with physician orders
- Provide therapeutic interventions to improve strength, coordination, mobility, and self-care skills
- Educate residents, families, and caregivers on treatment goals, progress, and home safety strategies
- Document evaluations, progress notes, and discharge summaries in compliance with facility, state, and federal regulations
- Collaborate with an interdisciplinary team including Physical Therapists, Speech Therapists, nursing staff, and physicians
- Ensure compliance with Medicare, Medicaid, and Skilled Nursing Facility regulations
- Maintain a safe and supportive therapy environment
- Current California Occupational Therapist license in good standing
- Graduate of an accredited Occupational Therapy program
- Experience in a Skilled Nursing Facility or post-acute setting preferred (new graduates encouraged to apply)
- Strong clinical, communication, and documentation skills
- Ability to work independently and as part of a multidisciplinary team
- Knowledge of Medicare guidelines and SNF documentation standards preferred
