As an Occupational Therapist, you will assess and treat residents to improve function, independence, and overall quality of life. This role includes developing evidence-based treatment plans, delivering skilled therapy services, and collaborating closely with the interdisciplinary team.
Key Responsibilities- Evaluate residents to determine Reciprocal impairments and therapy needs
- Develop and implement individualized treatment plans focused on ADLs and IADLs
- Track progress and modify treatment plans as needed in coordination with the IDT
- Recommend adaptive equipment and home modifications to promote independence
- Educate patients, caregivers, and families on therapeutic strategies
- Ensure accurate, timely documentation per state, federal, and facility guidelines
- Participate in team meetings and care planning activities
- Adhere to all regulatory and facility policies
- Active Texas Occupational Therapist license
- In good standing with all applicable licensing boards
- SNF or long-term care experience preferred
- Familiarity with Net Health is a plus
- Strong communication and interpersonal skills
