Psychiatric Rehabilitation Program (PRP) Coordinator

Reports To: PRP Director

Employment Type: Part-Time/Full-Time, Flexible Hours

The PRP Case Worker at mindful healing works provides Psychiatric Rehabilitation Program (PRP) services to adults and minors with priority mental health diagnosis, supporting skill-building, community integration, and recovery-oriented goals. This role involves a combination case collaboration, individualized rehabilitation planning, documentation, and group facilitation. The ideal candidate is flexible, client-centered, and comfortable working both independently and as part of a multidisciplinary team.

Key Responsibilities

Program Coordination & Client Support

  • Provide support and coordinate care for all PRP clients assigned to caseload in accordance with program standards and client needs.
  • Maintain ongoing communication with clients, families, internal team members, and external agencies.
  • Collaborate across all MHW programs to ensure high-quality psychiatric rehabilitation services.
  • Develop and maintain knowledge of and relationships with community resources.
  • Provide outreach and follow-up to maintain consistent client participation in services.

Direct Service Delivery

  • Provide assistance with self-care skills, social skills, independent living, cultural engagement, medication monitoring, wellness, employment, housing, and entitlement program applications.
  • Deliver individualized psychiatric rehabilitation services in home, community, office, and virtual settings consistent with program requirements—minimum 6 sessions/month per adult client and 3 sessions/month per minor client.
  • Facilitate at least one monthly PRP group, including preparation, engagement, and service delivery.
  • Participate in initial assessments and contribute to the development and implementation of Individual Rehabilitation Plans (IRPs).
  • Monitor client progress and adjust interventions as needed to support engagement and goal attainment.

Documentation & Compliance

  • Complete accurate and timely clinical documentation in accordance with COMAR standards, Medicaid requirements, and agency policy.
  • Maintain up-to-date treatment plans, progress notes, monthly summaries, and service encounters that reflect rehabilitation activities and client progress.
  • Conduct regular chart reviews and track client outcomes.
  • Uphold client confidentiality, HIPAA standards, and ethical practice.
  • Participate in supervision, team meetings, and ongoing training to maintain competency and compliance.
  • Collaborate regularly with multidisciplinary team members, including clinical supervisors, therapists, medication providers, and other PRP staff.
  • Participate in outreach and community education efforts to generate referrals and enhance visibility of MHW programs.
  • Communicate effectively about client needs, progress, coordinated supports, and emerging concerns.
  • Notify MHW prior to accepting any external employment opportunities.
Qualifications

Education

  • High school diploma or equivalent required.
  • Bachelor’s degree in Psychology, Social Work, Human Services, Rehabilitation Counseling, or a related field preferred.

Experience

  • Minimum of 1-2 years Experience working with individuals with persistent mental health needs or in a human services or community support setting strongly preferred.

    Experience in community mental health, case management, or psychiatric rehabilitation strongly preferred.
  • Group facilitation experience is a plus.

Core Competencies

  • Recovery-oriented, trauma-informed, strengths-based approach to service delivery.
  • Strong communication skills and the ability to build rapport with diverse individuals and families.
  • Ability to provide support in areas such as self-care, social skills, wellness, employment readiness, housing, and benefits navigation.
  • Basic knowledge of community-based resources and services.

Documentation & Organizational Skills

  • Ability to produce timely, accurate clinical documentation in compliance with regulatory and billing requirements.
  • Strong time management, organization, and prioritization skills to manage a flexible, hybrid caseload. and maintain compliance with agency standards.

Technical Skills

  • Proficiency in electronic health records (EHRs), telehealth platforms, and standard office software.
  • Comfort delivering services in person and virtually.

Logistical Requirements

  • Reliable transportation and ability to travel within the community to provide services.
  • Availability to work one weekend day, evenings and holidays, based on client, MHW and Clinical needs.
  • Commitment to a flexible hybrid schedule of at least 3 days on-site or in the community each week.
