Job Summary:

Med Star Health is looking for a Population Health Equity Outreach Coordinator to join our team! The Population Health Equity Outreach Coordinator Engages Population Health attributed patients for health risk assessments, annual wellness visits, chronic care management, data gathering, and assists to meet KPI goals for the department. Educates the public about programs and resources that promote physical and mental health, develops and implements public health programs, marketing and public awareness campaigns.

Primary Duties
  • Conducts outreach to Population Health patients to schedule Health Risk Assessments, Annual Wellness Visits, and Chronic Care Management services.
  • Gathers and manages patient data, including demographics and medical history, while performing registration and pre-visit tasks.
  • Schedules patient visits efficiently, ensuring adherence to policies and maximizing provider availability.
  • Composes patient correspondence and prepares reports to support Population Health initiatives and communication.
  • Contributes to process improvement and helps achieve departmental KPI goals through innovation and collaboration.
Qualifications
  • High School Diploma GED (Required)
  • Bachelor’s degree in public health, Health Administration, or related field
  • 1-2 years of experience in population health, managed care, or quality management programs (Required)
  • 1-2 years of experience working with diverse populations and providing culturally competent care (Required)
  • Licensed by the State of Maryland (Required)
Compensation

USD $20.57 - USD $36.27 /Hr.
