Position: Registered Nurse - Assistant Nurse Manager (Primary Care)
Demonstrates specialized knowledge and is responsible and accountable for all elements of the nursing process when providing or supervising direct patient care. Assesses, plans, implements, and evaluates delivery of care based on age specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care.

This position is open to current permanent employees of the Syracuse VA Medical Center and associated community-based outpatient clinics only.

The Assistant Nurse Manager is responsible and accountable for the provision of leadership in evaluating and improving patient care through collaborative strategies with others. They are responsible for advising the Nurse Manager of issues on the unit and potential resolution of issues. The Assistant Nurse Manager functions as an effective change agent, utilizing effective communication skills to implement improvements in patient care.

At the direction of the Nurse Manager, the Assistant Nurse Manager initiates interdisciplinary projects to improve organization performance and evaluates the effect on patient care and workload utilizing professional standards of care and practice. In the absence of the Nurse Manager, the Assistant Nurse Manager maintains accountability for professional nursing practice in assigned areas. The Assistant Nurse Manager possesses the knowledge and skills to effectively apply all aspects of the nursing process within a collaborative and interdisciplinary practice setting.

Responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements and evaluates care based on age-specific components. Adheres to established policies, standards of care and standards of practice. Assumes responsibility for the coordination of care focused on patient transition through the continuum of care, patient and family education, patient self-management after discharge, prevention and wellness based on the patient's goals, and supporting factors that impact customer satisfaction.

Considers all characteristics of the individual, including age and life stages, state of health, race and culture, values, and previous experiences. Administers medications and procedures per establishing policies and guidelines. Responsible for collaborating with services internal and external to the VA to facilitate care transition to manage and coordinate care to effectively meet the patients' needs. Provides consultation and education to nursing staff in areas such as new equipment, mandatory review, orientation and cross-training, and continuing education.

Demonstrates clinical expertise and knowledge of health care programs to improve the quality of patient care and the use of resources to facilitate compliance with internal and external requirements and standards. Demonstrates leadership by serving as an advocate for patients. Acquires and demonstrates strength in the areas of interpersonal relations, critical thinking, problem solving, and conflict resolution. Core elements of performance include knowledge and active participation in unit/program level quality improvement processes and initiatives as well as customer service programs, and any other duties as assigned.

The Assistant Nurse Manager executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care. The Assistant Nurse Manager is an expert nurse, with a vast background of experience, who has an intuitive grasp of each situation and focuses accurately on the problem without wasteful consideration of a large range of unfruitful, alternative diagnoses and solutions.

They demonstrate performance and leadership that is broad enough to improve the care for a group of patients and they are responsible for the documented outcomes at the program or service level. Program or service level outcomes must be broad and complex and can be demonstrated at any organizational level within a facility, VISN, or VACO. Program is identified as a substantial coordinated group of activities impacting patients and/or employees.
