• Directs patient care under the supervision of a Physical Therapist, performing assessments and interventions as ordered by the patient’s physician(s), within scope of practice. Participates in interdisciplinary care coordination and care planning.

• Accurately assess patients and collaborate with Physical Therapist to provide necessary interventions.
• Coordinate care with assigned Physical Therapist weekly and as needed for any change in patient condition.
• Follow established plan of care for all assigned patients and perform interventions per plan.
• Participate in interdisciplinary care planning for all assigned patients.
• Promptly report all changes in patient condition to the Case Manager RN or Physical Therapist and the patient’s physician(s) as appropriate.
• Provide cost‑effective and efficient physical therapy with regard to patient visits and medical supplies.
• Provide patient/caregiver education as per individualized plan of care and evaluate effectiveness of the plan, reporting any problems related to education.
• Complete all documentation within specified time frames.
• Appropriately manage time and daily tasks to achieve daily productivity goals.

• Graduated from an accredited Physical Therapist Assistant program.

• Minimum one (1) year experience in a Joint Commission accredited hospital. Minimum one (1) year experience in an acute rehab facility or equivalent serving the adult population. Six (6) months home‑care experience preferred. Computer skills preferred.

• Current license as a Physical Therapist Assistant in the state of Missouri.
• American Heart Association Basic Life Support certification.
• Current driver’s license and declaration of automobile liability insurance.

• Ability to stand and walk for long periods; walk up and down flights of stairs.
• Ability to receive, express, and comprehend detailed information orally; visual acuity; read and understand written directions.
• Excellent interpersonal and organizational skills.
• Ability to lift, transfer, and move patients weighing 10‑200+ pounds.
• Ability to safely operate an automobile.

• Frequent exposure to communicable disease via blood and body fluids.
• Exposure to infectious diseases and hostile persons at times.
• Works with sharps and other potentially biohazardous materials.
• Variation in environmental temperatures.
• Exposure to various community environments.
