Overview
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or Physician Assistant who operates independently, collaborating with facility staff to manage patient care in PALTC settings.
Responsibilities- Provide comprehensive assessments, diagnose, treat, and prescribe medications for acute and chronic conditions.
- Develop personalized care plans, promote preventive services, and monitor patient progress.
- Work autonomously within scope, making clinical decisions and escalating when needed.
- Build strong relationships with facility leadership, interdisciplinary teams, patients, and families.
- Ensure compliance with regulations, maintain accurate records, and participate in quality initiatives.
- Participate in infection control, health education, and safety protocols.
- Demonstrate strong communication, clinical decision-making, and proficiency with electronic health records and telemedicine tools.
- Require a relevant degree, minimum 2 years’ experience (long-term care preferred), and board certification.
This full-time role is based in-person across statewide PALTC facilities, offering comprehensive benefits.
