Provides competent clinical nursing care consistent with professional standards. Reporting and accountable to the Nurse Manager/Director, the Clinical Nurse is responsible for coordinating and delivering patient care utilizing the nursing process in a home health setting.

• Education – participates in identifying and meeting learning needs of self, attends education programs, uses patient education materials, assesses readiness to learn and developmental level of patient and factors affecting learning.
• Evaluation of care – identifies expected patient outcomes, seeks guidance for plan of care revision, participates in interdisciplinary team discussions and communicates relevant information to promote continuity of care.
• Implementation – demonstrates competence in care of patients with complex problems, utilizes organization and nursing standards, prioritizes care and consults with resources for complex care issues.
• Leadership – develops goals for professional growth, achieves or renegotiates goals, supports intra/interdepartmental collaboration, promotes cost‑effective care, gives feedback and maintains a safe, clean and secure environment.
• Patient assessment – recognizes data from complex situations to determine care priorities, considers physical, psychosocial, education and safety needs, anticipates discharge needs and develops relationships with families.
• Planning – collaborates with patient/family to prepare or update the plan of care, uses multidisciplinary resources, addresses cultural and ethnic issues and prioritizes tasks for complex patients.
• Research – reads relevant journals, brings ideas to staff for applicability.

Graduation from a school of professional nursing.

Licensed to practice as a Registered Nurse in the state of North Carolina.

One (1) year of nursing experience caring for acute and chronically ill patients or an equivalent combination of education and experience.
