Responsibilities

As a key member of our clinical team, you will provide routine skilled nursing care in the home, working under the direction of a Registered Nurse (RN). This role demands independence, strong organizational skills, and a commitment to communication. Our cutting-edge clinical point-of-care system provides real-time patient data to support your efficient workflow.

  • Executing the patient s plan of care as established by the RN.
  • Monitoring patient conditions and promptly reporting any changes or significant family situations to the RN, Supervisor of Clinical Services, and the attending physician.
  • Consulting with and seeking guidance from the RN as needed.
  • Participating in the development, implementation, and ongoing evaluation of the patient care plan.
  • Preparing, administering, and accurately recording medications according to agency policies.
  • Engaging in patient/family teaching and participating in case conferences.
Required Qualifications
  • Current and active CT Licensed Practical Nurse (LPN) License in good standing.
  • Graduate of an approved LPN program.
  • A minimum of 1 year of clinical nursing experience.
  • Valid driver’s license in good standing and reliable transportation.
  • Strong organizational and communication skills.
  • Ability to accurately follow and execute instructions.
Preferred Qualifications
  • Previous home care experience.
  • Experience with electronic documentation/point-of-care systems.
  • Proof of satisfactory completion of a medication administration course.
