As a Registered Nurse Home Health RN, you will play a key role in our mission to provide the Right Care, at the Right Time, in the Right Place for every patient and family.
Responsibilities- Assess physical, psychosocial, and environmental patient needs and develop a comprehensive plan of care.
- Coordinate, implement, and evaluate the plan of treatment, including primary responsibility for the caseload.
- Perform initial and follow‑up evaluations, document nursing findings, and revise the care plan as needed.
- Communicate significant findings and changes in health condition or unsafe facility conditions to the Clinical Supervisor, physician, facility, and other team members.
- Maintain patient and staff privacy and confidentiality in compliance with HIPAA and other applicable regulations.
- Collaborate with interdisciplinary team members to ensure continuous, high‑quality care delivery.
- Current, unrestricted state RN license.
- At least 1 year of clinical nursing experience, preferably in home health or a related setting.
- Associate’s or Bachelor’s degree in Nursing preferred.
- Valid driver’s license, dependable vehicle, and current auto insurance required; ability to travel up to 50% of the time.
- Previous home health or hospice experience desired.
- Strong communication skills and a passion for delivering compassionate patient care.
