• Developing and supporting integrated care plans with SPMI population in community setting
• Coordinate somatic & behavioral health care through communication with internal & external providers
• Utilize nursing experience and clinical knowledge to educate clients and staff on techniques to better manage their somatic conditions

• Bachelor’s Degree from an accredited School of Nursing
• Active Maryland RN licensure required
• 2 years of relevant work experience plus one year of supervisory experience preferred

• Pay Range - annually based on experience
