Join the Montgomery County’s Area Agency on Aging-Support Planning Agency’s team of committed professionals who aim to provide Medicaid reimbursed services according to each individuals health and safety needs. The Support Planning Agency program offers individuals services intended to allow them to remain in non-institutional settings for as long as possible. The Supports Planner assists with program enrollment, the development of plans of service and the monitoring of participants services.
The Support Planner, Aging & Disability (Program Specialist II) will:
- Provide case management (supports planning) services to participants of the Community Personal Assistance Services (CPAS), Community First Choice (CFC), Home and Community-Based Options Waiver (HCBOW) and Increased Community Services (ICS) programs. Supports planning consists largely of assisting applicants and participants with accessing Medicaid and non-Medicaid home and community-based services (HCBS). Case management services also include assisting applicants and participants with waiver eligibility, determination, and maintenance.
- Develop and submit an annual plan of service outlining the client’s services and providers based upon the annual assessment. Develop and submit a revised service plan when a change in service is needed.
- Contact clients at least monthly and conduct quarterly visits to assess the client’s functioning and monitor their services.
- Submit Reportable Events and enter billable activities into the online, electronic case record.
- Conduct Options Counseling for individuals in nursing homes who are seeking transition back into the community.
- Other duties may include assisting the supervisor as a lead worker/plan of service reviewer and covering for the supervisor by participating in Maryland Department of Health calls and meetings when the supervisor is unavailable.
The Support Planner, Aging & Disability (Program Specialist II) will need to travel throughout the County to conduct in-home assessments, conduct case management, and monitor services. A background investigation will be conducted into the selected candidate prior to appointment and will be a significant factor in the hiring decision.
Qualifications- Experience:
Two (2) years of professional case management experience with Medicaid Eligibility and Medicaid-funded Programs. - Education:
