Job SummaryThe Social Worker coordinates residents’ stays through admissions, discharge, and the MDS process. Responsibilities include crisis intervention, family communication, and professional social work assistance across illness, hospitalization, financial, and educational matters.
Job Requirements- Assists residents in exercising their rights and informs them of responsibilities related to those rights.
- Obtains and interprets information to meet residents’ needs.
- Demonstrates understanding of age‑appropriate care and adulthood health across the lifespan.
- Exhibits excellent written and verbal communication skills.
- Respects all members of the healthcare team.
- Possesses strong documentation skills.
- Maintains a solid work history with excellent attendance, punctuality, patience, and compassion.
- Provides considerate, respectful care focused on individual resident needs.
- Bachelor’s degree in social work from an accredited institution.
- Completion of a social work internship is required.
- LCSW/MSW with current active license in MD.
- Proven experience to perform daily tasks with an understanding of related duties.
