Full Time/Part Time Regular Full Time Total Hours Per Week 35 From Salary $54,000 To Salary $57,000
Job SummaryThe Care Manager is responsible for the Care Coordination of Targeted Case Management Cases. This includes assessment of new referrals, outreach to gain referrals and coordinating medical and mental health services with assigned Health Home.
Duties & Responsibilities- Perform CANS Assessment and Health Home Appropriateness Screening Assessment on all new referrals, determining qualifications for Health Home admission.
- Maintain all necessary Client consents for service.
- Conduct outreach in designated venues to generate program referrals.
- Utilization of web based:
Health Commerce System MAPP, UAS CANS and Health Home, GSI software. - Coordinate with Health Home on Consumer service needs.
- Demonstrate an understanding and put into practice the 5 elements of client Health Home service delivery.
- Conduct mandated monthly client contacts.
- Attend necessary appointments with clients.
- Develop client Plan of Care by the required due dates to include the DOH required 10 Care Plan elements.
- Follow all Health Home guidelines, policies and procedures.
- Attend all mandated and necessary Health Home and DOH training.
- Adhere to Agency contemporaneous documentation timeframe of 5 days from point of client contact.
Associate Degree in one of the following Fields:
Audiology, Child and Family Studies, Communication Disorders, Community Mental Health, Counseling, Education, Nursing, Occupational Therapy, Physical Therapy, Psychology, Recreation, Recreational Therapy, Rehabilitation, Social Work, Sociology, Speech and Language Pathology, Human Services, Human Development, Criminal Justice or other related degrees. One (1) years experience providing direct services to Severely Emotionally Disturbed (SED) children and adolescents, or providing linkage to services for children with one mental health diagnoses.
While performing the duties for this job, the employee is regularly required to communicate clearly and effectively. The employee is required to stand; use mobility, handle, feel; and reach. The employee is occasionally required to sit and stoop, kneel or crouch. The employee must occasionally lift and/or move up to 25 pounds.
