Shift: Full-Time | 12:00 PM - 8:00 PM | Every Other Weekend & Occasional Holidays

Anam Care and Anam Glen, a trusted assisted living memory care community in Rockford, is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our growing team. In this role, you will work closely with residents living with Alzheimer s disease and other forms of dementia, helping provide a safe, supportive, and dignified environment.

Primary responsibilities include, but are not limited to:

• Administer medications and monitor resident responses
• Conduct nursing assessments and monitor changes in resident condition
• Complete accurate and timely daily charting and documentation
• Supervise and support caregiving staff
• Assist with and support the community s memory care programming
• Communicate effectively with residents, families, and team members
• Respond appropriately to medical emergencies or disaster situations
• Ensure residents receive high-quality, compassionate care

• Active RN or LPN license in the State of Illinois
• 1+ year of experience working with Alzheimer s or dementia patients preferred
• Strong communication and teamwork skills
• Ability to multitask in a fast-paced care environment
• Compassionate, patient-centered approach to care
• CPR certification preferred (required after hire)

• Full-time or Part-time
• 12:00 PM - 8:00 PM shift
• Every other weekend required
• Occasional holidays
