Job Description
We are seeking an experienced  Home Health Registered Nurse (RN)  to provide high-quality skilled nursing care to patients in their homes. The RN will manage a daily caseload, perform comprehensive assessments, and deliver patient-centered care in compliance with home health regulations.

Job Duties

Provide skilled nursing care to patients in the home setting, including assessment, treatment, and patient education.

Perform comprehensive patient assessments and monitor clinical status, reporting changes as needed.

Administer medications and IV therapy and manage wound and cardiac-related care.

Complete accurate, timely documentation in compliance with home health and regulatory standards.

Coordinate care with physicians and interdisciplinary teams to ensure safe, effective patient outcomes.

Requirements
Requirements

Minimum  1 year of acute care RN experience  within the past 3 years

Home health, IV therapy, ER, or oncology experience  preferred

Active  Illinois RN license

BLS certification  required
