Community Support Team - Youth Case Manager

Provides mental health rehabilitation services and supports adults and families to decrease hospitalization and crisis episodes and to increase community functioning in order for the client to achieve rehabilitative, resiliency, and recovery goals. Provides continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participates as a team member in the delivery of mental health services to clients and their families and facilitates the integration of recovery into daily living.

The CST (Community Support Team) provides case management services, crisis intervention, skills training, documentation within required time frames, individual counseling, medication monitoring, attendance at meetings, quality improvement, and other duties necessary to meet the needs of individuals with serious mental illness.

• Bachelor’s Degree with a minimum of two years’ experience in mental health
• Demonstrated education, training, or experience in mental health services
• MHP designation
• Valid driver’s license

• Adequate written skills to accurately complete required documentation within prescribed time frames
• Excellent organizational and computer skills
• Self-starter with ability to work independently and in a team
• Skilled in conducting individual, family, and group counseling and case management
• Ability to present and coordinate lectures for clients, families, staff, or community resources

• Motivate and problem-solve with staff regarding effective clinical practices
• Be self-motivated, seek improved performance, and address complex client needs
• Adapt verbal and written communication to the language and style of the client and family
• Complete necessary documentation within established time frames
• Undertake primary case management responsibilities for designated clients
• Conduct individual, group, and family counseling and educational sessions
• Maintain client service records in compliance with regulations
• Maintain communications with all staff (admissions, processes, and discharge plans)
• Coordinate services from a variety of community resources
• Utilize a range of treatment concepts, evidence-based practices, and service modalities
• Present workshops, lectures, and trainings to clients, families, staff, and community groups
• Be familiar with court proceedings and available to appear on behalf of Rosecrance clients when needed
• Serve as a member of the Clinical Team and participate in all team meetings
• Maintain confidentiality per the Code of Conduct and applicable laws
• Deliver exceptional customer service to every client
• Model positive guest relations in representing the agency
• Valid driver’s license and ability to meet agency insurance requirements
• Must have own transportation

8-hour shifts. Monday-Friday with differing daily hours:
Monday-Thursday 10:30 AM–7:00 PM and Friday 8:30 AM–5:00 PM.

Full-time

• Base Pay:
  Starting at $22.50/hour (depends on education, experience, and credentials)
