We’re seeking a Home Health PT to serve patients across Rockford and surrounding communities.

🧠 What you’ll do

• 🏠 Conduct comprehensive in‑home PT evaluations and build individualized therapy plans
• 💪 Deliver skilled therapy: therapeutic exercise, balance & gait training, manual techniques, patient/caregiver instruction
• 🔄 Monitor progress and adjust plans; collaborate with physicians and the interdisciplinary care team
• 👨👩👦 Educate patients and families on HEPs, safety, equipment, and independence at home
• 📝 Complete required EMR documentation (evaluations, visits, reassessments) and support safe discharge planning

🎯 What we’re looking for

• ✔️ Home Health or comparable clinical experience preferred
• ✔️ Strong assessment, communication, and documentation skills
• ✔️ Comfort working independently in varied home settings; collaborative with RN/OT/SLP/HHA teams
• 🎓 Paid, tailored training (~2 weeks) in Arlington Heights before you take your own caseload
• 📈 Real opportunities for growth and continued learning
• Also available: Part‑time / PRN / Per Diem / Temporary
