Work Schedule: 100% FTE, Day Shift Role. 7:30am-4pm. You will work at the UW Health Swedish American Hospital in Rockford.

- Graduate of an accredited College of Pharmacy. Required
- Residency training in pharmacy practice, drug information, informatics or pharmacy administration or equivalent. Required
- Must provide copy of diploma/degree for highest level of education obtained. Required
- Current licensure from State of Illinois or license eligible. Required
