OverviewThe Home Visiting and Community Navigator is part of the Crisis Nursery team and is responsible for supporting families by identifying their needs, connecting them with community resources, and helping them set and achieve personal goals. You will work closely with community partners to maintain a strong network of available services and supports.
Qualifications- Bachelor’s Degree in a social services field, OR some relevant college coursework plus two years’ relevant professional experience, OR high school diploma plus three years’ relevant professional experience.
- Experience working with youth, adults or families strongly preferred.
- Demonstrates knowledge of various social service, educational and community resources.
- Demonstrates effective written and oral communication skills; consistently writes clearly and concisely for various mediums and audiences.
- Demonstrates sound professional judgment and problem-solving ability.
- Demonstrates strong interpersonal skills with the ability to develop trust, collaboration and teamwork with colleagues and participants.
- Ability to attend in-person meetings and events as needed.
- Valid driver’s license, current auto insurance and access to a reliable vehicle required.
- Build and maintain strong partnerships with local organizations, schools, community centers, healthcare providers, housing resources, and other agencies within program communities.
- Collaborate with Family Support staff to understand family needs, set goals, and provide referrals to appropriate community services.
- Help families access resources, strengthen support systems, and expand their community networks.
- Motivate participants to explore and use educational, employment, and community opportunities to achieve their goals.
- Identify challenges that may affect participant progress and provide support to keep them engaged and focused.
- Maintain accurate and timely participant records, case notes, and documentation in agency databases.
- Collect and review participant data to help inform program improvements and service delivery.
- Prepare internal reports to track participant progress and overall program performance.
- Clearly communicate and ensure understanding of program policies and procedures with all participants.
- Provide case management services, including conducting developmental screenings, making referrals to Early Intervention programs, and supporting families in maintaining regular well-child checkups and immunizations.
- Support and help facilitate parent support groups to strengthen family engagement and community connection.
- Follow up with families to ensure successful access to referred programs and community resources.
- Deliver the Parents as Teachers curriculum through home visits, promoting child development, positive parenting practices, and school readiness.
- Compensation:
Hourly. Range is between $18.70-$20.50 per hour, with offers commensurate with experience; bilingual candidates may receive additional compensation. The salary range provided represents our current estimate at the time of posting and is subject to change. Final salary will be determined based on experience, education, qualifications, skills, certifications, and alignment with organizational needs. - Benefits:

Hybrid (remote/in office); must be within reasonable driving distance to Rockford, IL. - Schedule:
Full-time, hourly; general business hours with some flexibility required for on-call responsibility and special projects. - Professional growth:
