Nurse Case Manager - Hospice Registered Nurse
We are seeking a Registered Nurse to service our patients throughout the Rockhill area. The Nurse Case Manager plays a critical role in the patient's care team, visiting patients and facilitating appropriate provider interactions. The position requires travel to patients’ homes, long‑term care facilities, and initiating telemedicine visits, while ensuring quality care that complies with all applicable laws and regulations.
Service Area- Spartanburg/Rock Hill area
This is a full‑time, salary‑based position with 12‑hour or 8‑hour shifts. Reliable transportation is essential as daily travel is required.

- Facilitate receiving all medical records from patient’s primary provider and specialists.
- Review medical records and complete consents with patients.
- Enroll patients in Care Management if eligible.
- Initiate a Care Management Plan of Care.
- Capture all diagnoses at the highest specificity and ensure acceptance.
- Complete Annual Wellness Visits (AWVs) for provider review.
- Complete cognitive impairment screenings and Social Determinants of Health (SDoH) assessments.
- Complete Advance Care Planning (ACP) conversations with the provider.
- Evaluate for home health, hospice, palliative or consults with specialty division.
- Evaluate for RPM devices and other beneficial resources.
- Communicate and coordinate care across the care team.
- Reconcile prescribed medications, OTCs, vitamins, supplements, herbal remedies.
- Provide post‑discharge education.
- Evaluate for adaptive equipment and DME.
- Ensure safe environment for patients.
- Document and log Care Management activities accurately.
- Coordinate with physical/occupational therapies, home health, hospice representatives, and other team members.
- Facilitate visits with appropriate provider or specialty entity.
- Coordinate telehealth visits when necessary.
- Coordinate with hospice interdisciplinary team.
- Participate in IDG meetings when needed.
- Registered Nurse with valid license in good standing.
- Minimum of three (3) years of clinical experience (community settings preferred).
- Strong communication and collaboration skills with interdisciplinary care teams.
- Proficient written and verbal communication.
- Basic computer knowledge.
- Leadership skills and ability to manage multiple responsibilities.
- Flexible hours and willingness to travel; licensed driver with insured vehicle.
