RN Case Manager - BONUS

Benefits: $5,000 Sign On Bonus.
Schedule:
Monday – Friday, 8 am – 4:30 pm. On Call Required.
Territory:
Rock Hill.

- Respond to clinical referral information with care and expertise.
- Partner with the Clinical Director to assess eligibility and suitability of clients.
- Provide direct care and support to patients, ensuring their comfort and quality of life.
- Deliver top‑notch nursing care.
- Educate and advocate for patients and families.
- Participate in call duties.
- Current unencumbered RN license.
- Minimum one year experience as an RN in a medical‑surgical/acute‑care setting; hospice experience preferred.
- Computer proficiency, including EMR background.
- Current CPR certification if required by state.
- Valid driver’s license, vehicle insurance, and dependable transportation.
$70,000 – $80,000 per year (average pay range). Final compensation will be determined based on experience and qualifications.
