Scope and Purpose of Position:
Under the direct supervision of the Program Manager, the Case Manager provides case management services to Hope House Women’s Shelter clients during their temporary residence at the shelter. 35 hours per week (Mon-Fri – 8:30 a.m. – 4:00 p.m. – one day per week scheduled for later start date in order to see clients in the early evening).
Responsibilities:- Assess and identify clients' needs. Advocate for clients to ensure that services of referral are accessible and provided to clients.
- Develop individualized service plans for each resident and ensure periodic review of goals.
- Provide supportive and crisis counseling and guidance individually and to groups.
- Provide information and refer clients to relevant community social services for medical, mental health, substance abuse, educational, vocational, public assistance, etc.
- Maintain all records and documents related to interviews, counseling meetings, and subsequent progress/status reports per agency and contract requirements.
- Develop and maintain effective working relationships with community social service providers (e.g., Department of Human Services, Veterans Administration) to enhance service quality.
- Report observations to supervisor on trends of homelessness, service needs, and effectiveness of referrals.
- Participate in weekly meetings with Senior Resident Assistant and Program Manager.
- Maintain HMIS data entry (intakes, transfers, discharges, etc.).
- Compile monthly statistics and prepare required reports in a timely manner.
- Collaborate with staff for case management and programmatic services.
- Share 24‑hour on‑call responsibilities with the Program Manager and Senior Resident Assistant every three weeks.
- Attend staff meetings, in-service conferences, seminars, training programs, and other agency informational meetings as required.
- Complete all mandatory training (in-person and online) in a timely manner.
- Provide office coverage 1–2 times per week during lunch time if needed.
- Other duties as assigned.
- Bachelor’s degree in human services preferred (Social Work, Psychology, Sociology, Criminal Justice, etc.) with 2 years of case management experience required.
- Experience working with vulnerable populations and understanding barriers faced by those who are homeless.
- Concern and sensitivity toward vulnerable populations; knowledge of social service resources.
- Proficiency with Microsoft Office and standard data collection systems.
- Ability to collaborate effectively with external service providers.
- Certification in Standard First Aid and CPR.
- Hepatitis vaccination or declination (within ten days of hire).
- Strong organizational abilities, attention to detail, excellent communication and problem‑solving skills.
- Valid New York State Driver License for transport of residents if necessary.
