Location: City of Rochester

Case Manager role at The Center for Youth Services, Inc.

Hours: Monday, Tuesday, Thursday, Friday: 9am-5pm;
Wednesday: 11am-7pm.

Overview

The Chrysalis House Case Manager will be assigned pregnant or parenting clients from both the residential and non‑residential tracks of the Transitional Living Program. This position will provide intensive case management, including basic and independent living skills assessment, training, referrals, and other supports as needed, as well as assist in implementing the parenting training and engagement across the TLP Program. This is a twelve‑month, full‑time position.

  • Develop, coordinate and facilitate parenting skills curricula, incorporating CYS philosophy and effective teaching approaches;

  • Outreach, screening, intake and assessment
  • Coordination, development, implementation, ongoing assessment of client case‑management needs, services and plans

  • Transporting program participants, as required and consistent with agency policies

Essential Functions
  • Manage the environment to provide a safe, supportive residential program through mediation, role‑modeling and positive interaction with Chrysalis House residents
  • Possess sound problem‑solving and decision‑making skills
  • Maintain clear professional boundaries
  • Work evening hours and weekends as needed
  • Promote strength‑based service delivery (youth leadership, asset development)
  • Be skilled in case‑management techniques and interventions
  • Be familiar with the homeless/runaway youth service system and community resources
  • Maintain accurate and up‑to‑date client records

Qualifications

A minimum of a Bachelor’s degree in human services, education, or related field; and three years’ experience or an equivalent combination of education, employment and life experience. A Master’s degree is preferred.

Before hire, candidates must meet the following criteria:

  • Authorization for the agency to obtain driving record information
  • Proof of a satisfactory physical exam within last 12 months including PPD and annually thereafter
  • Valid driver’s license
  • Regular access to a motor vehicle
  • Automobile insurance at level of $100,000 per person/$300,000 per accident bodily injury and $50,000 property damage
Physical Requirements

Candidates must be autonomously mobile to provide required services.

Computer Skills Recommended
  • Microsoft Office – Advanced
  • Internet Explorer – Advanced

Salary: $22 - $24.50 per hour
