• Ensure compliance with DOH, CMS, and federal/state regulations

• Provide hands‑on oversight and promote clinical accountability

• Mentor, coach, and support RNs, LPNs, and CNAs

• Assist with recruitment, onboarding, and retention

• Foster a positive, collaborative, and accountable culture

• Maintain continuous survey readiness

• Lead and support DOH and CMS surveys and audits

• Implement plans of correction and drive compliance initiatives

• Collaborate with physicians, rehab, social services, and dietary teams

• Oversee individualized care plans for high‑acuity residents (ventilator, dialysis, sub‑acute rehab)

• Assist with staffing, scheduling, and resource management

• Track clinical metrics (falls, infections, readmissions, wound care)

• Lead QAPI initiatives and implement corrective actions to improve outcomes

• Active, unencumbered RN license in New York

• 3–5+ years of experience in Skilled Nursing / Long-Term Care

• Leadership experience (Unit Manager, ADON, or similar)

• Knowledge of DOH/CMS regulations, MDS processes, and reimbursement models

• Experience managing high‑acuity resident populations

• EHR proficiency (Point Click Care  or Sigma Care preferred)
