RN Case Manager I-HC

We are seeking a dedicated RN Case Manager to provide comprehensive nursing care for a set of assigned patients. The role involves assessment, planning, coordination of interdisciplinary services, and supervision of Private Duty Nurses (PDN) as applicable.

40 hours per week

• 1 year of nursing experience preferred.
• Prior home health, clinical and direct patient care experience preferred.
• Ability to work independently.
• Solid interpersonal, organizational and time management skills.
• Proficient computer skills.
• Must be able to document clinical notes and assessments within an electronic medical record.
• Ability to travel to required locations as needed.

• Identify and prioritize health problems based on assessment.
• Develop or implement an interdisciplinary Plan of Care based on identified needs, with patient input and collaboration with the attending physician and other team members.
• Manage and coordinate patient care, including clinically complex cases, ensuring efficient and effective delivery of services and community supports.
• Exhibit proficiency and accuracy in comprehensive assessment/documentation, including payer-required assessments such as OASIS and HIS.

• Diploma or Associate’s Degree in Nursing required;
  Bachelor’s Degree preferred.

• Level II: a minimum of 2 years of home care experience.

Licensed RN; certifications as required by department.

M - Medium work: exerting 20‑50 pounds occasionally, 10‑25 pounds frequently, or >10 pounds constantly; requires frequent walking, standing, or squatting.

$77,983.00 – $ per year.
