RN Case Manager I-HC
We are seeking a dedicated RN Case Manager to provide comprehensive nursing care for a set of assigned patients. The role involves assessment, planning, coordination of interdisciplinary services, and supervision of Private Duty Nurses (PDN) as applicable.

Attributes- 1 year of nursing experience preferred.
- Prior home health, clinical and direct patient care experience preferred.
- Ability to work independently.
- Solid interpersonal, organizational and time management skills.
- Proficient computer skills.
- Must be able to document clinical notes and assessments within an electronic medical record.
- Ability to travel to required locations as needed.
- Identify and prioritize health problems based on assessment.
- Develop or implement an interdisciplinary Plan of Care based on identified needs, with patient input and collaboration with the attending physician and other team members.
- Manage and coordinate patient care, including clinically complex cases, ensuring efficient and effective delivery of services and community supports.
- Exhibit proficiency and accuracy in comprehensive assessment/documentation, including payer-required assessments such as OASIS and HIS.
- Diploma or Associate’s Degree in Nursing required;
Bachelor’s Degree preferred.
- Level II: a minimum of 2 years of home care experience.
Licensed RN; certifications as required by department.
Physical RequirementsM - Medium work: exerting 20‑50 pounds occasionally, 10‑25 pounds frequently, or >10 pounds constantly; requires frequent walking, standing, or squatting.
Pay Range$77,983.00 – $ per year.
