SummaryThe Float – Licensed Practical Nurse (LPN) is responsible to float to ambulatory practices within primary care where a need has been identified. The Float – Licensed Practical Nurse (LPN) is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

Responsibilities- Conducts and documents a thorough observation of each patient’s medical status upon presentation and throughout the patient’s course of treatment.
- Assists with the patient’s treatment plan under the direction of the RN or provider.
- Provides routine nursing services for patients as directed by the RN or provider.
- Complies with the evaluation, treatment, and documentation guidelines of the department.
- Completes required documentation in an accurate and timely manner.
- Provides regular patient status updates to the appropriate personnel.
- Attends and participates in department/facility meetings as required.
- Acts in compliance with hospital, regulatory, and professional standards and policies.
- Maintains clinical competency by pursuing continuing education opportunities.
- Performs other duties as assigned.
- Maintenance of all adult and pediatric competencies within primary care.
- Graduate from an accredited School of Practical Nursing
- 1 year of experience required
- Valid New York State (NYS) driver’s license required
- Current New York State (NYS) LPN license required
- Current Basic Life Support (BLS) certification required
- Compassionate, warm and patient focused.
- Critical thinking skills, decisive judgment.
- Exceptional documentation skills and professional behavior.
- 3 Years of Experience in clinical setting, ambulatory experience preferred

LICENSES / CERTIFICATIONSBLS - Basic Life Support - American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED)
Physical RequirementsMedium Work – Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects;
Requires frequent walking, standing or squatting.
