Responsibilities- Patient Care:
Determine patient needs through rounds, chart review, patient/family interviews and team conferences; coordinate multidisciplinary and agency case conferences; advocate with care managers for insurance coverage approvals. - Referral Management:
Manage referrals related to patients at risk, determine intervention strategies, document actions, and ensure inappropriate referrals are channeled correctly. - Treatment Plan Development:
Coordinate assessments and develop care plans in accordance with social work policy; implement plans preparatory to discharge or initiate continued care plans.
- Manage referrals related to psychosocial needs and determine appropriate interventions; review census to identify high-risk patients not referred; document social work interventions.
- Assess patient needs and determine appropriate modes of intervention; conduct psychosocial assessments and coordinate care plans; evaluate level of care when indicated; participate in multidisciplinary conferences.
- Execute discharge/continued care plans in compliance with regulations; coordinate with payers to obtain coverage approvals; involve patient/family in treatment planning.
- Arrange transportation as needed and act as liaison with the community and external referral sources.
- Perform other duties as assigned.
- One year of experience in a healthcare setting preferred.
- Compassionate, warm and patient-focused; exceptional documentation and planning skills; excellent communication and interpersonal abilities.
- For Elder
ONE employment: one year of experience with frail elderly population.
- For hires on or before December 31, 2023: a Bachelor’s Degree in Social Work or related degree is required.
- For hires on or after January 1, 2024: a Bachelor’s Degree in Social Work is required.
Minimum Qualifications
- Minimum of a Bachelor’s degree in social work; OR
- Minimum of a Bachelor’s degree in a human-services related major (e.g., sociology, gerontology, psychology, rehabilitation counseling, special education) with one year in a healthcare setting as a supervised social worker.
- Official transcript or verification from registrar’s office will be accepted upon graduation; primary source education verification required within 90 days of start date.
