*** Responsibilities:
** The Behavioral Health Patient Navigator (BHPN) position is part of the Pediatric Behavioral Health and Wellness, Integrated Behavioral Health Satellite based in Golisano Children’s Hospital Pediatric Practice (GCHPP), which is a part of the clinical service enterprise of the Department of Pediatrics. GCHPP, a primary care practice, provides outpatient medical care and related- family based services to patients (0-21 years) who reside in the greater Rochester region.

The service accepts Medicaid, Managed Care Medicaid, and private insurance plans and offers a sliding fee schedule to families with low income. The Integrated Behavioral Health Satellite clinicians support: (1) the care of all patients through mental and behavioral health consultation, resource recommendation, and referral support, (2) the development of treatment and safety planning for identified patients, and (3) short-term solution focused treatment to patients and their families.

The BHPN ensures patients receive the best care available to them by guiding patients through all the various complexities of the healthcare system. The role supports GCHPP patients and their families through consultation regarding community resources, preparation and follow up on referrals, and participation in appointments and community-based visits when clinically indicated. The BHPN provides field-based case management services, may serve as a main point of contact for patients, builds strong relationships with patients in order to help patients stay engaged in behavioral and medical care and adhere to their care plan.

The BHPN Identifies critical resources, helps navigate through health care services and systems, and promotes patient health. The BHPN works closely with the care team, which includes medical (e.g., physicians, nurses), mental health (e.g., psychologists, mental health therapists) and non- clinical staff (e.g., care coordinators) to support positive patient health outcomes. Individuals must subscribe to UR Medicine ICARE values including creating a culturally competent environment by treating patients, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
** ESSENTIAL FUNCTIONS
*** Provide consultation to medical and mental health clinicians regarding community resources for their patients and patients’ families
* Meet with patients and/or caregivers to assist with paperwork, correspondence, etc., necessary to ensure linkage to appropriate resources; as well as assist with access to urgently needed resources (e.g., food, housing, medical care)
* Participate in evaluation and treatment appointments with mental health clinicians and patients/families as needed (e.g., to share information about community resources and programs), as well as with pediatricians and nurse practitioners as needed.
