Position: Empowerment Services Peer Support (CORE)

Overview

East House is a dedicated non-profit organization committed to supporting individuals on their journey to recovery. We provide comprehensive services, including residential support, counseling, peer support and community outreach to empower individuals to overcome mental health and substance use disorders and lead fulfilling lives. Our mission is to foster a supportive environment that promotes healing and long-term recovery.

Under the general direction of the Community Oriented Recovery and Empowerment (CORE) Services Clinical Supervisor and Directory of Recovery Support Services, the Empowerment Services Peer Support is responsible for providing non-clinical, peer-delivered services with focus on rehabilitation, recovery, and resilience. They are designed to promote skills for coping with and managing behavioral health symptoms while facilitating the utilization of natural supports and community resources for adults in the community.

This can include advocacy, outreach and engagement, self-help tools, recovery supports, transitional supports, and pre-crisis and crisis supports. The peer carries out all job responsibilities with fidelity and accountability to the Agencies and Program Mission, Vision, Values, Guiding Principles, and Strategic Plan.

Responsibilities
  • Assists clients in articulating goals for recovery and determining steps and resources needed to achieve them.
  • Identifies barriers to community integration and provides appropriate interventions aligned with client recovery goals.
  • Maintains communication with program staff regarding client concerns and progress.
  • Coordinates and schedules recovery-based activities, trainings, and groups.
  • Collaborates with community agencies to gather and maintain relevant resources.
  • Provides outreach and support to clients in the community.
  • Promotes client use of community resources through modeling and education.
  • Assists clients in developing self-advocacy and empowerment skills.
  • Maintains documentation per East House policies and funder standards.
  • Transportation of clients in a vehicle as large as a 15-passenger van is also required of the position.
  • Meets clients in the community at time and location convenient for the client in order to provide services.
  • Engages clients in shared decision making around service and recovery planning, reviews progress toward goals and objectives; updates the service plan as goals are met or other changes are needed
  • Communicates with other providers such as prescribers, treatments providers, case managers, or employment specialists, to ensure integrated, collaborative care.
  • Builds self-advocacy skills and raises awareness of existing social support and services with clients
  • Engages clients in the recovery process with personalized support, increases their sense of hope and purpose, and empowers the individual to achieve their goals make positive changes. This includes: validating the individual, relating their own recovery stories and practices, modeling a recovery lifestyle, assisting the individual with identifying activities and coaching the individual as they identify barriers to engaging in these activities, wellness coaching with a focus on whole health, and celebrating the individual’s efforts and accomplishments.
