Job Summary
As part of a multidisciplinary public health team, the Community Health Worker (CHW) promotes the health and well‑being of communities disproportionately affected by HIV. This role conducts targeted outreach, provides HIV/HCV/syphilis screening, delivers health education, connects individuals to PrEP and other prevention services, and ensures linkage and re‑engagement in HIV care for people who are newly diagnosed or lost to follow‑up.
The focus population includes individuals vulnerable to HIV acquisition and those unaware of their HIV status.
Duties/Responsibilities
1. Outreach and Engagement
* Conduct outreach in community settings, such as shelters, encampments, drop‑in centers, street outreach, and events.
* Build trusting relationships using consistent presence, active listening, and nonjudgmental engagement.
* Engage individuals unaware of their HIV status or needing support accessing prevention or care.
* Assist with community events, workshops, and pop‑up clinics.
* Use motivational interviewing to support informed decision‑making.
* Maintain partnerships with local organizations and coalitions.
* Utilize mobile outreach units for hard‑to‑reach areas.
* Learn and implement evidence‑based interventions (harm reduction, trauma‑informed care, etc.).
* Serve as an ambassador for all Trillium Health, MOCHA Center, and Harm Reduction services.
2. Service Delivery
* Conduct assessments to identify health needs, substance use patterns, and barriers to care.
* Support clients navigating the healthcare system, including scheduling and attending appointments.
* Provide person‑centered, culturally responsive, trauma‑informed support.
* Offer safer‑sex supplies and risk‑reduction counseling.
* Facilitate linkage to PrEP/PEP, HIV care, behavioral health, and supportive services.
* Support Rapid Start ART for newly diagnosed individuals.
* Re-engage individuals previously diagnosed with HIV but out of care.
* Assist with coordination and follow‑up to ensure clients remain engaged.
* Document services accurately and maintain confidentiality.
3. Testing
* Provide HIV, HCV, and STI screening in clinical, community, and mobile settings.
* Conduct pre‑ and post‑test counseling and deliver results.
* Support clients with individualized prevention or care plans.
* Initiate Rapid Start workflows for HIV and HCV.
* Provide condoms and harm‑reduction supplies.
* Develop working knowledge of HIV/HCV/STIs and related public health issues.
4. Health Promotion
* Empower clients through health literacy and informed decision‑making.
* Implement evidence‑based prevention programs.
* Collaborate with healthcare providers and community partners.
* Advocate for access to HIV prevention, PrEP, and medical care.
* Participate in coalitions and public health committees.
5. Administrative
* Compile and report program data in electronic systems.
* Participate in ongoing training and continuing education.
* Use Microsoft Office and digital platforms for communication and documentation.
* Work collaboratively with the healthcare team to maintain client engagement.
* Participate in quality‑improvement initiatives.
* Maintain positive communication and teamwork.
6. Miscellaneous
* Demonstrate commitment to inclusiveness, diversity, and cultural humility.
* Model One Trillium behaviors.
* Maintain confidentiality of all protected information.
* Develop knowledge of HIV, harm reduction, substance use, and LGBTQIA+ competency.
* Participate in audits and improvement plans.
* Perform other duties as assigned.
