Overview

Under the supervision of the Assigned Management, this position provides the quality care necessary to restore or maintain the patient’s physical and emotional well-being. The aide assists with the physical and personal care of clients and their activities of daily living, including assistance with meals, maintenance of safety and environmental upkeep. The aide maintains a supportive and empathetic environment for patients and families.

Responsibilities

Level I responsibilities:

  • Follow the Hospice Plan of Care for each individual patient as directed by the RN.
  • Observe patients’ conditions and report any changes to the RN.
  • Turn and reposition bedridden patients alone or with assistance.
  • Feed patients who are unable to feed themselves; serve and collect meal trays.
  • Measure and record food and liquid intake/output and vital signs; report changes to professional staff.
  • Assist patients with walking, exercising and moving in and out of bed, if needed.
  • Assist with personal care – bathing and grooming.
  • Answer patient call lights.
  • Give regular attention to the patient’s comfort and communicate perceived needs to the nursing staff.
  • Maintain a neat and clean patient area.
  • Change linens as needed.
  • Complete light housekeeping as necessary.
  • Stock/store medical supplies and linens.
  • Offer support and comfort to families at the time of the patient’s death.
  • Provide post‑mortem care.
  • Assist RN and other disciplines with health and non‑health related tasks.
  • Demonstrate high standards of integrity by supporting the company’s mission, values and corporate code of conduct.
  • Maintain high regard for member privacy in accordance with corporate privacy policies and procedures.
  • Maintain reliable attendance and punctuality.
  • Perform other functions as assigned by management.

Level II additional responsibilities (in addition to Level I):

  • Provide mentoring/support to other Home Health Aides as requested.
  • Provide hands‑on experience to new Home Health Aides.
  • Work with patients and their care givers who need additional support.
Qualifications
  • High School Diploma or equivalent.
  • Completion of Hospice Orientation within 3 days of hire before working as a Hospice Aide.
  • At least one year of Nursing Assistant or Home Health Aide experience.
  • HHA Certified or ability to meet one of the following eligibility requirements for competence:
    • Currently certified CNA.
    • Documented home health aide or nurse aide training and competency evaluation from an out‑of‑state training program.
    • Home health aide with documented training and competency evaluation who has not been employed as a home health aide for 24 consecutive months.
  • Must complete Lifetime’s competency program and earn active New York State HHA certification within 4 days from hire if qualifying with a CNA, an out‑of‑state training program, or as an HHA without 24 consecutive months of employment.
  • Interest and sensitivity in caring for the terminally ill.
  • Ability to read, write, understand, and carry out directions/instructions; maintain simple records.
  • Pre‑employment drug test and physical examination required; annual health assessment (as required by NYS Health Department) mandatory.
  • Respect patient confidentiality.
  • Accomplish duties within allotted time; work is thorough and neat.
  • Two years of Hospice Aide experience (Level II).
  • Participate in the training of new agency home health aides (CHHA) during initial training/certification.
  • Work independently and under minimal supervision.
  • Provide mentoring to new Hildebrandt Hospice Aides as requested.
  • Regularly complete ongoing hospice education through the online portal.
  • Complete Safety Food Handling Certification Level II after 6 months in the role.
  • Demonstrate a higher level of commitment to outstanding customer service internally and externally.
  • Supervisor recommendation is required.
  • Maintain good standing with no disciplinary action and positive performance measures.
  • Ability to interact, perceive, and anticipate the needs of the patient.
