Location: City of Rochester
HEALTH AND WELLNESS CASE MANAGER
Location: Rochester, NY
Employment Type: Full-Time, 40 hours per week (Mon-Fri 9am-5pm)
Salary: $20.00 - $22.00 per hour
Description: Provides intensive case management for newly-arrived resettlement clients identified as having special health needs. Works with department members to ensure vulnerable clients have access to mental health and specialized community services, supporting trauma-informed care.
Key Responsibilities- Maintain a caseload of clients and keep case files in accordance with agency, departmental, regulatory, and funder guidelines.
- Complete relevant screenings and assessments (e.g., psychosocial, mental health, financial, employment).
- Create, implement, and monitor comprehensive service plans and goals.
- Assist with discharge planning to ensure a safe transition.
- Facilitate transportation to appointments and accompany clients as needed.
- Assist clients in meeting expectations of service providers, courts, landlords, and employers.
- Inform clients of opportunities for social support and self‑sufficiency.
- Collaborate with referring agencies and internal staff to support client needs.
- Coordinate client benefits and monitoring of treatment needs.
- Report significant behaviors or events requiring immediate response.
- Advocate for resources and complete referral documents for public agencies.
- Maintain knowledge of community resources and link clients to services.
- Use databases to keep documentation compliant with regulations.
- Participate in agency meetings and ongoing professional development.
- Meet productivity and documentation standards to support organizational financial stability.
- Follow incident reporting processes and inform supervisor of significant events.
- Attend program, departmental, case review, and training meetings.
- Maintain professional knowledge through education and training.
- Stay updated on regulations governing client population.
- Engage in interagency meetings and communication with providers.
- Pursue professional competencies through training and conferences.
- Support the Agency’s mission, vision, values, and strategic plan, including core competencies.
- Education: Bachelor’s Degree in Social Work, Human Services, International Relations, or related field preferred. Equivalent combination of education and experience considered.
- Credentials: Valid NYS Driver’s License, Valid PPD screening available.
- Experience: 1–2 years of case management and community outreach with experience in substance use disorder, geriatrics, mental health, and refugees. Knowledge of Medicaid, SNAP, Social Security, and legal systems helpful.
- Physical Demands: Ability to sit, talk, hear, stand, walk, use hands to type, and perform light lifting. Close and distance vision required. Quiet noise level in work environment.
