General DescriptionProvide assistance and instruction in activities necessary to promote and maintain independence for individuals with intellectual/developmental disabilities who choose to self‑direct their services. The Self‑Directed Program provides as‑needed care to individuals as assigned to meet their daily needs and enable them to function at the highest possible level. Effectively supports ongoing and collaborative communication among all program staff and is responsible for ensuring a trauma‑informed care environment.
EssentialDuties And Responsibilities
- Assist individuals with specific facilitative outcomes as described in each individualized Self‑Directed Plan, including personal care, community integration, money management, advocacy, employment needs, and respite.
- Continuously seek opportunities to meet individuals’ psychosocial needs through care planned and spontaneous actions.
- Provide reliable, safe, high‑quality services as directed in a thorough and caring manner.
- Implement a person‑centered approach to services.
- Follow universal precautions, proper infection control, sanitation, and safety standards of practice in all work and activities.
- Complete paper timesheet/invoice and web‑based time keeping in a timely manner in accordance with payroll processing.
- Communicate with supervisor and Circle of Support about individuals’ habilitation plans that may need revisions or updates.
- Communicate effectively with individuals with intellectual/developmental disabilities, the Circle of Support, broker, fiscal intermediary coordinator, service coordinator, supervisor, interagency professionals, community professionals, and families.
- Consistently work with all team members, led by individual and family preference, to meet the needs and desires of the individual served.
- Respect the individual’s dignity, privacy, property, religion, and culture.
- Document and maintain accurate records of contacts with individuals.
- Provide appropriate interventions for all crisis situations and immediately notify the appropriate person of any allegation of abuse or neglect.
- Provide transportation, as needed, to individuals and follow safety procedures regarding transportation.
- Maintain flexibility in schedule based on needs of individuals assigned.
- Be dependable and arrive on time, ready for responsibilities.
- Participate in relevant agency meetings and/or trainings.
- Actively pursue development of professional competencies related to job role performance through reading, supervision, in‑service training, and attendance at conferences and workshops.
- Be aware and actively support the agency's mission, vision, values, and strategic plan, including the agency's core competency initiatives of collaboration, community focus, innovation, and cultural competence.
- Perform other duties as assigned by the individual being served or CCFCS SDS supervisor.

- Education:
High School Diploma or equivalent preferred; relevant combination of education and experience will be considered. - Credential:
If driving is required, a clean and valid NY State Driver License and access to a reliable and insured vehicle is required; agency requirements dictate that anyone whose job description includes driving responsibilities must be at least 21 years of age. - Experience:
At least one year of practical experience working with individuals with intellectual/developmental disabilities preferred. - Physical Demands/Work Environment:
