Job Description

Under general direction of the Highland Women’s Health (HWH) Nurse Manager and Division Director, the OB RN Care Manager’s central role integrates and coordinates access and utilization management, proactive patient management, care facilitation and treatment planning functions for the obstetric patient population throughout the HWH practice. The RN Care Manager facilitates clinical aspects of a patient‑centered medical home, working with the interdisciplinary medical team in the provision of patient care and may also participate in supervision of other clinical staff members.

• Licensed RN in NYS; BSN preferred.
• Three to five years of professional nursing experience, preferably outpatient OBGYN nursing.
• Progressive clinical experience; community health, care management and/or disease management preferred.
• Experience working as part of a multidisciplinary team (required).
• Supervisory experience desirable.
• Experience in data analysis preferred.
• Licenses/Certifications:
  
  fetal monitoring certification (NCC or PQF) current or obtained within 6 months of hire; current CPR certification (AHA BLS or ARC CPR); successful completion of medication proficiency examination; valid New York State driver’s license and ability to travel.
