Nutritionist

We are seeking a compassionate and knowledgeable Nutritionist to join our team. This position plays a vital role in assessing and supporting the nutritional needs of patients receiving home care services.

As a Nutritionist, you will provide in-home assessments and education to patients, developing tailored nutritional plans that promote health and improve outcomes. You’ll collaborate closely with nurses, home health aides, and other care professionals to ensure each patient receives high-quality, coordinated care.

• Conduct comprehensive nutritional assessments for patients in their homes
• Develop individualized meal and nutrition plans based on patient diagnoses, dietary restrictions, and goals
• Educate patients and caregivers on proper nutrition, food preparation, and dietary management
• Monitor and evaluate patient progress, adjusting plans as needed
• Collaborate with the clinical care team to support overall treatment plans
• Participate in care team meetings as required

• NYS Certified or Registered Dietitian/Nutritionist
• Minimum of 1 year experience in clinical or home health care setting
• Excellent communication, teaching, and interpersonal skills
• Able to work independently, manage time effectively, and travel to client homes as needed
• Bilingual candidates a plus

Compensation: $30.00 - $35.00 per hour
