Job Summary

Demonstrates knowledge of human growth and development, assessment, range of treatment, and care of patients appropriate to the ages served. Promotes customer service initiatives and Hospital Values. Documents all patient care, including CARE scores. Utilizes the Nursing Process for individual care to meet patient needs and performs technical skills and procedures in compliance with National Patient Safety Goals.

Essential Functions
  • Performs duties in accordance with unit‑specific standards of care and evidences compliance through documentation, observation and competencies.
  • Completes patient assessment within 24 hours of admission and each shift as needed.
  • Develops Nursing Plan of Care within 24 hours of admission and updates it at least every 24 hours.
  • Forms and implements nursing actions, recording responses in Meditech.
  • Involves patient/family in care when appropriate and plans/implements patient teaching.
  • Respectfully addresses patients’ spiritual and cultural needs.
  • Recognizes, documents and reports changes in patient status and adverse responses to care.
  • Communicates hand‑off information at shift change, between departments, upon transfer and discharge.
  • Provides patient education and documentation of understanding regarding tests, procedures, equipment and medications.
  • Manages pain through assessment, intervention and re‑assessment within one hour.
  • Implements orders as directed, including medication administration and reconciliation.
  • Meets physical and psychosocial needs for an assigned group of patients and prioritizes care.
  • Delegates patient care within scope of practice and directs LPN or Nurse Technician staff as appropriate.
  • Delivers medication safely, including eMAR scanning where available.
  • Adheres to policies and procedures concerning safety, precautions and use of safety devices.
  • Participates in process‑improvement activities.
  • Maintains patient confidentiality, privacy and HIPAA compliance.
Qualifications
  • High School Graduate.
  • Associate’s or Bachelor’s Degree or higher in Nursing from an accredited school.
  • Registered Nurse licensed in New Hampshire or appropriate compact licensure.
  • Basic Life Support (BLS) required;
    Certified Registered Rehab Nurse (CRRN) preferred.
  • 1–2 years’ experience in an acute care or rehab setting.
  • Working knowledge of medical terminology.
  • Excellent computer skills, including Microsoft Office.
