Monroe Plan for Medical Care is hiring Care Managers in the Albany area. Join our team of dedicated, caring professionals in our passionate pursuit of improved access and quality of healthcare for underserved populations.
Responsibilities- Care Management – Receive referrals, conduct comprehensive assessments, develop person-centered plans, coordinate services, maintain regular contact, provide home visits, and travel as required.
- Documentation – Complete all required documentation in a clear, concise, and timely fashion; maintain up-to-date assessments and care plans in the electronic record.
- Collaboration – Work as a member of the multidisciplinary Care Management team, facilitate meetings with members, providers, and family, and participate in inter‑agency teams.
- Communication – Present professionally, provide feedback to providers, engage members, and deliver program information as needed.
- Master’s degree in a Human Services field and 1 year of experience with serious mental illness, developmental disabilities, or substance abuse; OR Bachelor’s degree and 2 years of such experience; OR credentialed Alcoholism and Substance Abuse Counselor (CASAC) with 2 years of experience.
- Demonstrated ability to respect diversity and maintain confidentiality.
- Strong teamwork, communication, organizational, and computer skills.
- NY‑state driver’s license and reliable transportation for travel to member homes.
- Previous experience as a Health Home Care Manager.

The minimum and maximum annual salary for this position is $46,948 – $57,380. In addition to salary, Monroe Plan offers a comprehensive benefits package and non‑monetary perks.
Candidate must be willing to travel throughout the Rochester and surrounding area and have experience working with adults.
