Support Worker - Crisis Resolution Home Treatment Team (CRHTT) | NHS AfC:
Band 3 | Permanent, full/part-time or job share options. 37.5 hours per week on a rota (24/7 pattern). Site posts available at Tameside, Stockport, Oldham, Rochdale and Bury. Salary £24,937 - £26,598 per annum, pro rata. Closing date: 15/02/2026 23:59.

Main duties of the job

As a Support Worker in the CRHTT, you will assist qualified practitioners in delivering intensive, 24-hour support to service users experiencing acute mental health crises, offering an alternative to hospital admission. You will help service users engage with community resources, promote independent living, provide practical and emotional support, and contribute to safeguarding and relapse prevention. The role involves working a rolling rota covering both day and night shifts.

You will work within a multidisciplinary team and participate in training and development.

Key responsibilities include:

  • Assist qualified practitioners in delivering care within the Crisis Resolution Home Treatment Team.
  • Support service users to engage with appropriate services and resources (benefits, employment, housing, substance misuse services).
  • Identify safeguarding concerns for adults/children and escalate appropriately.
  • Monitor signs of relapse and communicate with the multidisciplinary team.
  • Promote independent living and provide health promotion information and support.
  • Support monitoring of physical health and mental state; maintain accurate documentation per Trust policies.
  • Support service users to engage in education, employment, and vocational activities; involve families and friends as appropriate.
  • Contribute to safeguarding, relapse prevention, and risk management processes.
  • Participate in meetings, supervision, and training; maintain confidentiality at all times.

Qualifications and experience

Education/Qualifications
  • Good standard of general education including numeracy and literacy skills.
Experience
  • Experience in a supportive role and with individuals with mental health needs or in distress.
  • Experience of lone working and coordinating activities for service users.
Knowledge
  • Safeguarding knowledge and ability to support vulnerable adults; understanding of mental health signs and escalation; awareness of local services and benefits/employment systems; ability to work with people in crisis and part­ner with third sector organisations.
Skills and Abilities
  • Ability to implement care plans with team support; empathy for service users; ability to work unsupervised in various settings; calm, professional response to emergencies; willingness to undertake Venepuncture and physical health checks training.
