The Nurse Practitioner coordinates the planning, implementation and ongoing monitoring and evaluation of patients of all age ranges, under the supervision of a collaborating licensed physician. Responsibilities include examining, diagnosing, treating patient conditions, educating patients and family members about medical options, prescribing medications and treatments, and performing other duties to support patients.

• Work independently with other members of the medical team that take part in an interdisciplinary team that provides primary care to employees and patients.
• Prescribe medications following the clinic’s policy, formulary and relevant regulations of the State.
• Performs and documents mental health assessments, consults as needed with the provider/practitioner concerning the plan of care.
• Maintains current knowledge of community resources, how to access them and the regulations for access to them.
• Communicates effectively in both the patient chart and orally with the staff so as to ensure that can be understood and executed by others as appropriate.
• Complies with all established safety procedures to ensure a safe environment for patients, visitors and staff.
• Participates in performance improvement activities.
• Performs other duties as assigned.

• Master’s degree in Nursing from a state‑accredited school required.

• Current IL APRN Licensure.
• Current IL DEA License Required.
• BLS Required.

• 2 years Family Practice experience preferred.

• Strong computer skills with the ability to navigate EMR programs.

• Be compassionate, sensitive, and possess strong reasoning and thinking skills.
• Ability to work independently, prioritize and complete tasks in a timely manner.
• Excellent ability to thrive under pressure and adapt to changes in assignment.
• Strong communication skills including reading, writing and the ability to listen effectively.
• Ability to work within the confines of confidentiality and maintain professional boundaries.
