Working with Lifesprk’s Care Access team at North Memorial, Care Access provides around the clock clinical support and community resources for patients, families, caregivers and the North Memorial health system. The RN collaborates with medical providers and clinical teams to promote patient and family wellness, improve care outcomes and access to appropriate resources. Using the nursing process, clinical judgment and established protocols, the RN also provides telephone triage: assessment, problem identification, interventions and evaluation.

• Collaboration with patients, families, and care providers to deliver appropriate care resources
• Telephone triage to collect data, form a nursing assessment and interventions using established protocols
• Recognition of emergent situations and responding promptly and effectively

• Bachelor's degree in nursing preferred.
• Graduate from an accredited school of nursing.
• 1-2 years nursing experience in a hospital, nursing home or clinic setting (3 years preferred).
• Experience with care coordination and telephonic triage preferred.
