- Integrates health care for clients from pre-admission to post discharge, coordinating and sequencing the client’s care.
- Enhances patient/resident flow and interdisciplinary communication to promote early intervention.
- Evaluate outcomes based on integration of clinical, financial, and utilization data.
- Operate in structured and unstructured health care settings with varying policies and procedures.
- Utilize independent nursing judgment when integrating health care and adhere to age-specific care methodologies across neonatal to geriatric populations.
- Work with growth and development needs of client populations; communicate with patients/residents, family members, and the health care team.
- Coordinate, facilitate, and negotiate with others; monitor and trend patient/resident responses using structured care methodologies.
- Exercise fiscal responsibility and manage variances/duplication.
- Demonstrate independence, assertiveness, critical thinking, clear documentation, problem solving, and self-motivation; work with minimal direction.
- Incorporate teaching/learning principles and adapt teaching to age groups; facilitate groups and implement projects as needed.
- Establish priorities, maintain flexibility, and organize effectively; possess knowledge of reimbursement for services.
- Work primarily day hours, scheduled weekends, and occasional evenings; may be exposed to communicable diseases, hazardous materials, and injury; subject to multiple sensory and environmental stressors.
- Bachelor's degree in Nursing strongly preferred; graduate from an accredited nursing program (CCNE, ACEN, or NLN CNEA).
- Minimum of two years’ clinical experience preferred.
- Currently holds an unencumbered RN license in the practicing state and/or multistate licensure if in an NLC state.
- Obtains and maintains required department-specific competencies and certifications; depending on location, a valid driver's license may be required.
